For children aged twelve to fifteen (12-15), a sworn affidavit by the parent that a child does not have criminal record is required\n\t\t\t\t\t\t\t\t\t<\/li>\n<li>\n\t\t\t\t\t\t\t\t\t\tColour copy of driver\u00e2\u20ac\u2122s licence\n\t\t\t\t\t\t\t\t\t<\/li>\n<li>\n\t\t\t\t\t\t\t\t\t\tColour copies of all identity documents\/books or cards\n\t\t\t\t\t\t\t\t\t<\/li>\n<li>\n\t\t\t\t\t\t\t\t\t\tColour copies of all military service and discharge documents, if applicable\n\t\t\t\t\t\t\t\t\t<\/li>\n<li>\n\t\t\t\t\t\t\t\t\t\tColour copies of all documents evidencing a name change (other than through marriage), if applicable\n\t\t\t\t\t\t\t\t\t<\/li>\n<li>\n\t\t\t\t\t\t\t\t\t\tColour copies of documents related to source of funds\n\t\t\t\t\t\t\t\t\t<\/li>\n<li>\n\t\t\t\t\t\t\t\t\t\tEight (8) passport-size photos\n\t\t\t\t\t\t\t\t\t<\/li>\n<li>\n\t\t\t\t\t\t\t\t\t\tProof of residential address (lease agreement \/ title deed and utility bill)\n\t\t\t\t\t\t\t\t\t<\/li>\n<li>\n\t\t\t\t\t\t\t\t\t\tColour copies of university\/college diplomas\n\t\t\t\t\t\t\t\t\t<\/li>\n<li>\n\t\t\t\t\t\t\t\t\t\tLetter of recommendation from the head of the school\/university for children between 12 and 18 years old. Official transcripts from an institution of higher learning or a letter from the competent authority confirming enrolment for children aged 18 to 30 who are currently in a recognised institution of higher learning\n\t\t\t\t\t\t\t\t\t<\/li>\n<li>\n\t\t\t\t\t\t\t\t\t\tNotarised affidavit of support for each dependant aged 18 or over included in the application, with the exception of the spouse\n\t\t\t\t\t\t\t\t\t<\/li>\n<li>\n\t\t\t\t\t\t\t\t\t\tHIV test results (for all applicants aged 12 or above)\n\t\t\t\t\t\t\t\t\t<\/li>\n<li>\n\t\t\t\t\t\t\t\t\t\tRoutine blood and urine test results\n\t\t\t\t\t\t\t\t\t<\/li>\n<li>\n\t\t\t\t\t\t\t\t\t\tOne (1) professional reference\n\t\t\t\t\t\t\t\t\t<\/li>\n<li>\n\t\t\t\t\t\t\t\t\t\tLetter of employment\/financial statements\n\t\t\t\t\t\t\t\t\t<\/li>\n<li>\n\t\t\t\t\t\t\t\t\t\t12 months bank statements\n\t\t\t\t\t\t\t\t\t<\/li>\n<li>\n\t\t\t\t\t\t\t\t\t\tBank reference letter\n\t\t\t\t\t\t\t\t\t<\/li>\n<li>\n\t\t\t\t\t\t\t\t\t\tNotarised affidavit of source of funds\n\t\t\t\t\t\t\t\t\t<\/li>\n<li>\n\t\t\t\t\t\t\t\t\t\tNotarised Statutory Declaration\n\t\t\t\t\t\t\t\t\t<\/li>\n<li>\n\t\t\t\t\t\t\t\t\t\tDetailed business background reports\/resume\/CV\n\t\t\t\t\t\t\t\t\t<\/li>\n<li>\n\t\t\t\t\t\t\t\t\t\tLetter to the Minister\n\t\t\t\t\t\t\t\t\t<\/li>\n<li>\n\t\t\t\t\t\t\t\t\t\tProof of payment of relevant fees\n\t\t\t\t\t\t\t\t\t<\/li>\n<\/ul>\n<p>Any colour copies of documents must be notarised and legalised before they\u00e2\u20ac\u2122re submitted. If any documents are not in English, they must be translated, notarised, and legalised prior to submission.<\/p>\n<p>The main applicant and adult dependants must sign forms where a signature is required. For adult disabled children lacking the capacity to sign forms, appropriate legal documentation must be provided by the person signing the forms demonstrating that person\u00e2\u20ac\u2122s legal right to sign on behalf of the dependant.
